INTEGRATED CARE & SPECIAL PROGRAMS
COMMUNITY CONNECTIONS A program that supports families with school-age children struggling with behavioral health challenges. A trained Family Liaison assists the family with coordinating a team of professionals to meet the needs of the family in a family-centered approach.

SUMMIT SAFE HAVEN This region’s only Mental Health Triage and Detoxification Center serving adults and adolescents who are experiencing a mental health or substance abuse crisis. Summit Safe Haven provides a safe, structured program that is available 24 hours a day, 7 days a week with a strong case management program and follow up services.

INTEGRATED CARE PROJECT In partnership with the Summit Community Care Clinic, Colorado West offers an integrative care approach that addresses whole person health encompassing the behavioral and physical needs of their clients. This innovative approach is a nationally recognized program and has been operating in Summit County for over 6 years.

STAFF
Program Director     Kathryn Davis, LPC
Kathy joined Colorado West as Summit County Program Director in 2007 with over 20 years in management and administration. Kathy obtained her Bachelor’s degree in Psychology from Regis University, and her Master’s degree in Counseling Psychology and Counselor Education from the University of Colorado/ Denver.   Kathy is a certified instructor of Mental Health First Aid, a founding member of the 5th Judicial Drug Court Team and avid bicyclist.

Psychiatrist     Kevin Scott Jones, MD 
Dr. Jones joins the staff of Colorado West having practiced Adult Psychiatry at Maricopa Integrated Health Systems in Phoenix, AZ. Having obtained a Bachelor of Science degree in Biology from Presbyterian College, Dr. Jones graduated from Ross University School of Medicine. A native of South Carolina, Kevin enjoys playing tennis, baseball and the theatre.

Licensed Clinical Supervisor     Jeanette Kintz, LPC, LAC  
Jeanette has been with Colorado West since 2002, when she started as an intern in her Master’s program at University of Colorado/ Denver.  She specializes in mental health, addictions, trauma, and clinical supervision.  She is also largely involved with the Summit County Drug Court as a treatment provider.  She enjoys biking, reading, scrapbooking and spending time with her young daughter.

Licensed Clinician     Kirstie Taylor, LCSW
Kirstie, a Licensed Clinical Social Worker, has been with Colorado West since 2007 and holds a Master’s degree from University of Denver.  Kirstie specializes in crisis intervention, grief and loss, case management and aging services.  She loves snowboarding, mountain biking, swimming and hiking with her dogs – Spyro and Bronco.

Licensed Clinician     Susan Toys, LPC, LMFT
Susan joined the staff of Colorado West Mental Health in Frisco in 2008. She has been a Licensed Professional Counselor and has practiced marriage and family therapy since 1989. Additionally, Susan has enjoyed facilitating psycho-educational workshops, participated in the Summit County Pro Bono Jail Therapy Program, and taught high school English.

Licensed Clinician     Dan Ireland, LPC, LAC
Dan joined the treatment team at Colorado West in 2009 bringing with him almost 25 years experience. Dan specializes in treating people who are experiencing acute or chronic episodes of mood disorders, anxiety and problems with substances including alcohol and other drugs. In addition to receiving a Master’s degree in Counseling from the Denver Seminary, Dan is a Licensed Professional Counselor and a Licensed Addiction Counselor. Dan enjoys fly-fishing, hiking, reading, football and photography.

Licensed Clinician     Victoria Hamilton, LPC
Victoria holds a BS in Psychology from Colorado State University, and a MA in Clinical Counseling from the University of Northern Colorado. Victoria uses EMDR techniques in conjunction with other traditional methods of therapy to help people who suffer from trauma, anxiety, panic, disturbing memories, post-traumatic stress, and many other emotional problems.  Victoria is a Colorado native and enjoys spending her time outdoors with her wonder dog Murphy.

Licensed Clinician     Sadie Smith, LMFT
Sadie joined Colorado West in the Fall of 2012 after working for Yellowstone Behavioral Health Center in Cody, WY for 3 years.  Sadie holds a B.M.Ed from VanderCook College of Music, Chicago, IL and an MA in Counseling Psychology; MFT from Regis University, Las Vegas.  Sadie is a licensed marriage and family therapist, and a certified EMDR therapist specializing in treating children ages 0-5.  Sadie also enjoys working with: trauma, depression, anxiety, substance abuse disorders as well as adolescents and family issues.  Sadie enjoys finding new adventures traveling the third world.          

Clinician     Rachel Forbes, MA
Rachel is a bi-lingual therapist (Spanish speaking) who holds a Master’s degree in Social Work, with a concentration in International and Community Development from Monmouth University.  She also holds a Bachelors of Arts degree in both Spanish and Psychology from Loyola University Maryland.  Her professional interests include sustainable development, homeless youth advocacy, and community gardening for wellness.  Rachel is also a classically trained pianist who enjoys teaching in her spare time.

Community Connections Family Liaison     Sadie Keller, MA
Sadie comes to Colorado West after working in community mental health and crisis on the front range of Colorado. She holds a Master’s degree in Transpersonal Counseling Psychology, Wilderness Therapy, from Naropa University. Sadie’s focus is working with families in need of community resourcing as well as crisis intervention. Sadie is an outdoor enthusiast who enjoys all kinds of recreation in her personal time as well as incorporating nature and activity into her working relationships whenever possible.

Detox Supervisor     Cindy Peratt
Cindy has been with Colorado West since 2011.  Cindy brings over 20 years of teaching and administrative experience to our team. She earned a BA degree in Communications with a minor in Spanish, and completed a MA degree in Educational Administration from the University of Colorado. Cindy’s strengths include proven skills in administration, conflict resolution and grant writing. She is currently pursuing Certified Addictions Counselor certification. Cindy enjoys hiking, back country skiing and recently rekindled her interest in distance running after completing the Steamboat Marathon.

Medical Case Manager     Trevor Baringer
Trevor has been the Medical Case Manager for the Frisco clinic since 2011. He has a Bachelor of Science degree in Psychology from the University of North Dakota. His laid back personality and willingness to help others has made him an excellent resource for clients.   He enjoys hiking, snowboarding, spending time with his family and staying active with his dog, Thor.

Business Services     Carrie Tellevik
Carrie joined Colorado West in 2010.  Carrie obtained her Bachelor of Arts degree from The Evergreen State College in Olympia, WA.  Carrie excels in customer service and creating a welcoming environment for the clients.  She enjoys, camping, hiking, skiing, and taking long walks with Jake, her border collie/lab dog.

Business Services     Lisa White
Lisa joined Colorado West in 2011.  Her position as a Business Services Assistant enables her to combine her business organizations skills with her desire to be of service to others.  A twenty year resident of Summit County, Lisa enjoys hiking, camping and practicing Reiki.

Business Services     Kellie McConnell
Kellie has been with Colorado West as Business Services Assistant since Fall 2012 coming to us with over 20 years of customer service experience. Kellie is a California native and has been living in Summit County for 14 years.  She is married to an amazing Fire Fighter in Breckenridge and stays very busy with three wonderful daughters.  She enjoys hiking, camping, traveling and volunteering in the community.
